An incident is any occurrence that results in or may have resulted in hurt, disease, property damage, or even death. A "Near Miss" occurrence could have resulted in harm, but it was avoided.

Our compassionate and skilled support staff is dedicated to assisting you with daily personal tasks to make you feel safe, comfortable, and entirely supported. Our products include:
- The participant's passing.
- Participant suffering a serious injury.
- Mistreatment or disregard for a participant.
- Violence or unlawful physical or sexual contact with a participant.
- Sexual mistreatment or participant grooming.
- The unapproved application of a participant-involved restrictive practice.
Ensuring Safety and Preventing Incidents
Your safety is our first concern at Authentic HealthCare Services Treatments, and we work hard to provide safe treatments. But we know mistakes and accidents can happen. These events are what we call "incidents." It is essential that you, as a participant, help prevent incidents by being conscious of what you do and considering others around you, even though we strive hard to prevent them. If something is bothering you, please speak up.

If you are a party to an incident, you have the right to be treated with dignity and respect. You are entitled to know what went wrong and what steps are being taken to correct it. You can file a complaint if your problems have not been addressed.
